The role of race and environment in the development of hyperosmolar hyperglycaemic non-ketotic coma.

چکیده
Introduction It has been reported that hyperosmolar hyperglycaemic non-ketotic coma (HHNC) occurs more commonly in black patients (Pyke, 1969) and this has become an accepted part of clinical teaching on the subject (Oakley, Pyke and Taylor, 1978; Jackson and Vinik, 1977). Recently this point has been further emphasized (Nikolaides et al., 1981; Wright et al., 1981). Our personal observations however suggested that poor social circumstances might be more important than racial factors in the aetiology of HHNC. A retrospective analysis of patients with the condition presenting to Dudley Road Hospital, Birmingham, a district general hospital serving a substantial immigrant population, was thus undertaken.

The syndrome of hyperosmolar hyperglycaemic non-ketotic diabetic coma was first described by Sament and Schwartz in 1957. In a recent publication, McCurdy (I970) reviewed the details of 84 cases. Various possible aetiological factors have been considered, and in an Editorial in journal of the American Medical Association (I968), attention was drawn to an iatrogenic element. It has complicated p...

متن کاملHyperosmolar non-ketotic diabetic coma and rhabdomyolysis.
Rhabdomyolysis is a rare but potentially fatal complication of hyperosmolar states. We report a case of severe hyperosmolar non-ketotic diabetic coma causing rhabdomyolysis in a young man. Despite very high levels of creatine kinase there was no detectable myoglobinuria. Creatine kinase estimation should be a standard investigation in all patients presenting with a hyperosmolar state.

Generally, cardiac arrest due to pulseless electrical activity has a poor outcome, except when reversible factors such as acute hyperkalaemia are identified and managed early. Hyperosmolar diabetic non-ketotic coma may lead to acute hyperkalaemia. Hyperosmolar diabetic non-ketotic coma is a metabolic emergency usually seen in elderly non-insulin dependent diabetics, characterized by severe hype...
